Community Service Volunteer Hours
1)Read through the Graduation Requirement & Student Volunteer brochure so that you understand the Volunteer Service requirements for graduation as well as the optional service hour opportunities available,
a) If you do not have a copy of this year’s “Service Learning Graduation Requirement and Student Volunteer Service Program” brochure, you may download it by clicking here for the page1 and here for page2
2)“Student Volunteer Service Application and Approval Form”
FOR EACH ORGANIZATION, PRIOR TO YOUR VOLUNTEERING SERVICE HOURS…
a) You, the student, complete PART A
b) Your Parent/Guardian completes PART C
c) The volunteer coordinator/director or individual supervisor of the project or agency where you will be volunteering your service hours completes PART B
d) Mail “Student Volunteer Service Application and Approval Form” to, Service Program Coordinator, Broward Virtual School, 6600 SW Nova Drive, Fort Lauderdale, FL 33317 (keep a copy for your documentation)
e) If you do not have a copy of this year’s “Student Volunteer Service Application and Approval Form” brochure, you may download it by clicking here
3) If the Organization is approved, log your Service Learning Requirement hours on the Tier I form. If you need additional space, you may download or make additional copies of the form –to download, click here
4) For each organization where you complete service hours, mail your original completed/signed Tier I Log Sheet to Service Program Coordinator, Broward Virtual School, 6600 SW Nova Drive, Fort Lauderdale, FL 33317 –keeping a copy for your documentation
5)Along with your 40 hour Service Log, you need to submit a one-page reflection of your service learning requirement.
This writing requirement is part of the American Government curriculum. Whether you did this already through your American Government class, or if you need to do it separately, please mail your written reflection to the BVS Service Program Coordinator along with your Service Log(s)THE ONE-PAGE WRITTEN REFLECTION & 40 SERVICE HOURS ARE REQUIRED
6) If you choose to pursue service hours beyond the required 40 hours (to pursue the Florida Academic Scholars Program or the Silver Cord), you need to log your additional hours on a different form, “Student Volunteer Service Program, Tier II/Tier III Log Sheet”
a) You may download this form as well, click here
b) Remember that you need to have completed serving your required 40 service hours first
c) In addition to logging your service hours on one side, you must also complete the reverse side of the form
d) If you need additional space, you can make additional copies of the form
e) Note: The 40 required service hours counts toward the additional 75 or 250
7) When your goal of 75 or 250 service hours is documented, mail your original completed/signed Tier II/Tier III Log Sheet to, Service Program Coordinator, Broward Virtual School, 6600 SW Nova Drive, Fort Lauderdale, FL 33317
